No Surprises Act
You have the right to request a Good Faith Estimate of our charges before scheduling services.

The No Surprises Act is a federal law that entitles you to an estimate of the cost of services at our practice. However, Ohio licensing board regulations require us to provide the actual cost of services in a written informed consent form before treatment begins. You will receive this information prior to your appointment and before any charges are incurred.

It is often difficult to determine the exact number of sessions required, as this depends on an evaluation of your concerns and the progress you make, which is influenced by your engagement in the process. You may choose to discontinue services at any time, or services may be terminated per the terms outlined in the informed consent form.

While the No Surprises Act allows you to dispute charges that significantly exceed the Good Faith Estimate (by $400 or more), this situation is unlikely. State regulations require that you agree to the actual session costs in advance. Dispute resolution information is available upon request. Any fee adjustments will be reflected in a revised informed consent form, which must be agreed to before taking effect. Otherwise, fees remain in place for 12 months.
